Event form templates
Event forms are template-based on purpose: you pick the use case, PRYVC generates the fields and the consent disclosure, and the language ships versioned — every submission records the exact text it was shown. You never edit the disclosure, which is precisely what makes it defensible.
Each mockup below shows the form as a visitor sees it after scanning your booth QR, with example recipients. Your business name, event, and brand color replace the samples automatically.

What every template shares
Section titled “What every template shares”- Custom fields: up to five extras (short text, choice, checkbox) on any template.
- Locked disclosures: the consent block is generated from your recipients and channels; the copy is versioned, and each submission records the exact text hash it was shown.
- The receipt: after submitting, the visitor sees a confirmation with their record hash, a public-ledger link, and how to revoke — no PRYVC account needed.
- Ephemeral data: submissions are encrypted on arrival and purged once delivered to you (how delivery works).
